1 /4 /2008 Madison An employee was fatally injured when he was backed over and crushed by a
bulldozer.
1 /4 /2008 Columbus An employee was fatally injured after falling from approximately 30 feet off the cab platform
of an overhead bridge crane. The employee was believed to have lost his balance while locking out the crane's control power and fell over a slack single chain guardrail.
1 /12/2008 Cleveland An employee was caught and crushed to death between a straddle forktruck
and a rack of steel.
1 /18/2008 Cal City An employee was fatally injured when he was struck by a cement truck while directing traffic. The employee was not wearing a traffic vest nor using signs or paddles to stop traffic.
1 /24/2008 Milwaukee An employee was electrocuted while aiding in the process of loading concrete
hoppers onto a truck. The employee was not aware that the crane was in contact with the overhead
power lines, when he took grip of a metal chain sling from the crane to rig it to the hoppers.
1 /25/2008 Peoria An employee was asphyxiated after being exposed to hydrogen sulfide while attempting
to drain an oil well.
1 /26/2008 Toledo An employee was fatally injured after falling approximately 28 feet from a 8/12 pitch
residential roof. At the time of the accident the employee was in the process of installing
a roof vent. Snow and ice were present on the roof and contributed to the fall.
1 /28/2008 Cal City An employee was fatally injured after falling approximately 15 feet through a fiber glass roof.
At the time of the accident the employee was repairing the roof.
1 /28/2008 Chicago North An employee was overcome by carbon monoxide in a car wash equipment room.
The sources of the employee's exposure may include a defective exhaust system on the boilers
in the room.
2 /5 /2008 Chicago North Employee was fatally injured when he apparently fell 8 feet in a dark storage room.
Employee was found dead by a co-worker, but there were no direct witnesses.
2 /10/2008 Peoria Two employees were fatally injured when overexposed to carbon monoxide.
Apparently, an air handling unit which contained a charcoal filtering unit, began smoldering.
Eleven other employees were treated at various levels for CO exposure.
2 /16/2008 Madison An employee was fatally injured when he was crushed in the cab of an overturned backhoe.
The employee was operating the backhoe moving 12,000 lb river weights on a oil pipeline installation
jobsite when the backhoe overturned. River weights are concrete weights used to hold pipes down in marshy areas. The weights were being moved using the bucket of the backhoe.
2 /20/2008 Columbus An employee was fatally injured when he was crushed between a coil of steel
and the metal base used to support the coils during an annealing process. The employee
was in the process of re-torquing bolts on the metal base when a 33,900 pound coil was placed down
on top of him.
2 /27/2008 Peoria An employee was fatally injured when he was struck by a ruptured tire and/or a single
piece rim wheel. Employee was using a leverage bar to mount the tire of the bead.
2 /27/2008 Cincinnati An employee was fatally injured when the accelerator of an all-terrain vehicle he
was operating got stuck in the full throttle position and drug him approximately 50 feet before
slamming into a block wall.
3 /7 /2008 Aurora Employee was fatally injured when he was engulfed by soybeans
in a grain bin.
3 /8 /2008 Toledo An employee was fatally injured when she was caught between the stand-up forktruck
she was operating and a storage rack. The employee was backing the forktruck up when she ran
into a storage rack and was pinned between the forktruck and the rack.
3 /10/2008 Peoria Two employees were fatally injured in a trench cave-in. The trench was approximately
9-10 feet deep, 11 feet wide,and consisted of type B soil. The initial cave-in trapped the first
employee and when the second employee entered the trench to try and rescue the first
employee, a second cave-in occurred trapping him.
3 /10/2008 Toledo An employee was fatally injured when he slipped and fell on the sidewalk at the entrance
of the building. The employee struck his head against a brick wall when he fell. It had snowed
early in the morning and the sidewalk had recently been plowed and shoveled.
3 /11/2008 Chicago North An employee was fatally injured when he fell approximately 50 feet through
a floor hole on the decking of an amusement ride that was being demolished.
3 /14/2008 Cleveland Employee was fatally injured when a 1868 lb steel plate fell from a crane.
At the time of the accident, the employee was attempting to load the steel plate into a press brake.
3 /17/2008 Peoria Employee was fatally injured when a mobile home collapsed on top of him.
The employee was in the process of jacking up the mobile home to prepare it for transportation
when the jack failed and the mobile home fell approximately 10 inches onto the employee.
3 /20/2008 Toledo An employee was fatally injured when he was struck in the head by a boring bit that
became dislodged and thrown from a running CNC machine.
3 /20/2008 Peoria An employee was fatally injured when the crane he was operating overturned
and struck the side of a flatbed trailer, crushing the cab. The employee was attempting to lift
two steel plates, weighing approximately 15,100 lbs.
3 /21/2008 Aurora Employee was fatally injured when he tripped and fell down two steps and struck
his head. The employee was turning off TV sets at a off-track betting site.
